    Role of HRT for Menopause

Looking out for the best medical approach to get relief from menopausal symptoms? Hormone replacement therapy (HRT) in Mumbai at Harleys Cosmetic & Women Clinic offered by the best gynecologist can be your best-suited choice!

Dr. Shilpa Bansal Agrawal, an expert gynecologist in Mumbai can help you manage the hormonal imbalance and need of hormone replacement by providing an effective treatment. She is known for offering the finest treatment against the hormonal imbalance in females after menopause.

The ovaries- a female reproductive organ, secretes two main sex hormones: estrogen and progesterone in every woman. Estrogen plays a role in many bodily functions including the thickening of the uterine lining to prepare it for the possible implantation of a fertilized egg. It can help influence the body to utilize calcium and prevent osteoporosis, helps keep the vagina healthy, and even can help maintain healthy blood cholesterol levels. The role of progesterone in the female body includes blood pressure regulation, improvement in mood and sleep, and preparation of the uterus for the implantation of a fertilized egg as well as maintenance of pregnancy.

    What is HRT?

As women begin to experience long transition periods leading to menopause, their ovaries no longer produce sufficient amounts of estrogen and progesterone hormones. The decline in these female sex hormones can lead to a range of physical and emotional menopause symptoms, including:

    • Vaginal dryness; pain or discomfort while having sex
    • Hot or cold flashes and/or night sweats
    • Dry eyes, skin, or mouth
    • Insomnia (trouble sleeping)
    • Urinary urgency (a frequent urge to urinate)
    • Mood swings, irritability, or mild depression.
HRT, Hormone Replacement Therapy is a treatment wherein the lower levels of female hormones, experienced by old women hit by menopause, are replaced to ease the above-mentioned symptoms of menopause. Whether or not this therapy must be undertaken is left to be decided by a healthcare provider, a gynecologist. Once menopausal women start with HRT, it is best to take a few months to see whether it really works for them. If not, the doctor can either provide a different type of HRT or increase the dose.

    Types of HRT

Hormone replacement therapy is mainly of two kinds:

    • Combined HRT in Mumbai

In this type of Hormone Replacement Therapy, the doctor prescribes menopause-affected women a combination of estrogen and progesterone/progestin (a synthetic form of progesterone).

The combined HRT may include the use of oral progestins: natural pills such as medroxyprogesterone acetate and synthetic progestin pills. Researchers suggest that natural progesterone is the right choice for women having high blood cholesterol as it may not affect lipids. Therefore, many gynecologists recommend natural progesterone pills for menopausal women instead of synthetic ones. Moreover, a gynecologist may even suggest the use of low-dose intrauterine devices containing progesterone when women enter perimenopause and ask women to keep it until this menopause transition phase gets over to help with their irregular periods.

Combined HRT is recommended for women who still have their wombs/uterus. It’s more beneficial than taking estrogen alone as it can help lower the risk of endometrial and uterine cancer. Generally, it is taken as a form of birth control but it can help provide relief from many menopausal symptoms.

    • Estrogen-only HRT in Mumbai

In this therapy, a gynecologist most often prescribes a low dose of estrogen to be taken daily in the form of a pill or a patch. The estrogen hormone may even be prescribed in the form of gel, spray, cream, or vaginal ring.

Estrogen hormone replacement therapy is recommended for women who have had their womb/uterus removed in a surgical procedure called hysterectomy or a hysterectomy with bilateral salpingo-oophorectomy- a surgery done to remove the uterus, fallopian tubes, and ovaries.

    Suitable Candidates for HRT

Hormone Replacement Therapy can be undergone by any woman experiencing menopausal symptoms. However, this treatment may not be suitable for every menopausal patient, or there might be a need to take a specialist/gynecologist’s opinion. The non-suitable candidates for HRT include those who:

    • Have liver disease
    • Have past medical records of heart disease or stroke
    • Have unexplained vaginal bleeding
    • Are pregnant or have suspected pregnancy or lactating
    • Have untreated hypertension- it is important to control blood pressure before commencing with HRT
    • Have a history of blood clots- in such a case HRT in oral forms is not recommended by HRT through the skin can be considered
    • Have a history of cancer of the female reproductive system including ovarian cancer, breast cancer, or uterine cancer.

In such circumstances, menopausal women can get relief from menopause symptoms through another alternative- like various other kinds of medications.
